The American Limb Preservation Society (ALPS) is proud to offer this nationwide directory of clinics that self-identify as practicing multidisciplinary limb preservation. This resource is intended to help individuals and families locate clinics committed to preventing amputation and preserving mobility through collaborative, evidence-based care.
Please note that this directory is not an endorsement or certification of any clinic by ALPS. Inclusion in this directory is based on self-reported information. We encourage patients and caregivers to use this tool as a starting point—and to actively evaluate the clinics they are considering.
To support your decision-making, we’ve developed a Patient Guide below with key questions to ask when choosing a limb preservation clinic. Quality limb preservation care is a team effort, and informed patients are essential members of that team.

Patient Guide: How to Vet a Limb Preservation Clinic
Finding the right clinic is critical to protecting your health, mobility, and quality of life. The following checklist is designed to help you ask informed questions and assess whether a clinic provides true multidisciplinary limb preservation care.
General Clinic Questions
- Do you have a dedicated limb preservation program or team?
- What types of specialists are involved in your limb preservation care? (Examples: vascular surgery, podiatry, wound care, endocrinology, infectious disease, prosthetics, behavioral health, dietitians, physical therapy)
- Are services coordinated in one location or across different facilities? Is Telemedicine available?
- How quickly can new or urgent limb-threatening conditions be seen?
- What insurances do you accept: (Examples: Medicare, Medicaid, Private Insurance Carriers, ACOs)
Team-Based, Multidisciplinary Care
- Do specialists work together as a team on my case? Generally, who leads the team?
- Will I have a single point of contact to help coordinate my care? Who is that typically?
- How do team members communicate with each other and me about my progress?
Clinical Expertise
- How many patients with diabetic foot ulcers, PAD, or limb-threatening conditions do you see annually?
- What are your limb salvage and amputation success rates?
- Do you participate in research, registries, or quality improvement initiatives related to limb preservation?
Wound and Vascular Care
- Is there access to advanced wound care therapies?
- Is vascular testing ABI onsite and routinely utilized?
- Does your center perform or coordinate revascularization procedures if needed?
- If needed, is coordination for home health care available?
Patient Support
- Do you provide patient education on foot care, diabetes management, ulcer prevention, etc?
- Is there access to social work, behavioral health, and care coordination services?
- If needed, are prosthetics and rehabilitation part of your care continuum?
Communication and Respect
- Will I be included in decision making about my care?
- Do you endorse seeking second opinions?
- Do you provide accessible communication for individuals with disabilities or language needs?
